Forms, Fees & Registration
To avoid late fees, which are paid by the athlete and not the JCC, it is imperative that all materials, forms, and fees be processed on time. Payments, confirmation kits, online registration, and medical forms turned in past the deadline will be charged a $125 late fee per item received past posted deadlines. Athletes who fail to turn in the required deposit and paperwork by the deadline are subject to replacement. Please adhere to all guidelines in order to avoid extra charges.

Online Registration
Athletes who are selected by a coach for a place on Team Houston will be required to register online. This is a JCCA national requirement and will be subject to a $125 late fee if not completed by the deadline of March 7. Online registration will be up and running by January and can be found at www.jccmacreg.org.
Confirmation Kits
Each athlete is required to complete a confirmation kit before attending the JCC Maccabi Games. The kit consists of signature page, individual sport form, sexual harassment policy, copy of health insurance, birth certificate, electronic photo, and medical form. With the exception of the medical form, all paperwork will be due by March 7 before a $125 late fee will be applied.
Payments
Payment can be made by cash, check, and credit card, or through automatic withdrawal. Balance must be paid in full by March 7, 2010.
