We are disappointed you have chosen to discontinue your membership at the J. As per your membership agreement, completion of this form fulfills the requirement of a 30-day written notice. Effectively, you will be responsible for one to two more billing cycles, regardless of the day of the month you submit this notice, in order to cover the remaining 30 days of your membership with a prorated amount due.
